FACILITY USE GUIDELINES
Facility Use Fees are due and payable to Bethlehem Lutheran Church ten (10) days prior to the use of the facility and staff services.
Children must be in the area where the parents or adult supervisors are meeting so there is full supervision at all times. Careful supervision provides safety for children.
Smoking or use of alcoholic beverages is not permitted on the premises.
Please report any malfunctioning equipment or damage to the property to church staff immediately.
A member of Bethlehem will be present to assist and monitor events during which the kitchen and dining hall are used.
Use of the elevator between the ground and basement level provides safety and access to events in the dining area.
Detailed instructions for weddings can be obtained by consulting with the Wedding Coordinator.
The organ and grand piano can only be used with permission . THE GRAND PIANO MUST NOT BE MOVED!!!!!
The sanctuary is limited to worship services, weddings, funerals, retreats, concerts and recitals approved by Bethlehem. No food or beverages are to be brought into the sanctuary.

AGREEMENT ACKNOWLEDGEMENT
Liability insurance, with Bethlehem Lutheran Church listed as an additional insurer, will be required for any community group larger than 15. This includes for-profit and non-profit groups. A certificate of such insurance will be required as proof for use of BLC facilities.
I have read the terms of agreement and understand that my submitting this form indicates an agreement with these terms. I agree that I am responsible for this event and that Bethlehem Lutheran Church is not liable for this activity or incidents arising from it.
